This is the Statutory Sick Pay regime of Great Britain; in the United States, ask your employer for its own absence form or an FMLA certification.

When do I need a fit note instead?
From the eighth calendar day of the absence. Up to seven days, weekends and days off included in the count, you certify yourself; beyond that your employer can ask for a fit note, which a nurse, pharmacist, physiotherapist or occupational therapist may issue as well as a doctor.
